Referral Info
Referral is required from your doctor when you attend the Clinic. Complaint referral (for medicare purposes) must include +\- ultrasound, +\- mammogram and +\- biopsies. Doctors’ name, provide number and signature.
Referral is only valid for 12 months so a new referral is required every visit.
